3 Copies of the label to be cooked. The capacity for which the label is to be registered — Required
4 Scanned copy of challan deposited for the prescribed fee for registration — Required
5 Name of the bottling unit, address and copy of the bottling license of the respective label — Required
6 Scanned copy of the Registration Certificate under the Companies Act — Required
7 Scanned copy of company PAN card — Required
8 Scanned copy of FSSAI license of the company — Required
9 GST Scanned copy of registration — Required
10 If the label is to be supplied by bottling from outside the state of Chhattisgarh, then the scan copy of the order of registration of that label in that state and the scanned copy of the registered label and the no-objection certificate of that state — Required
11 Supply of foreign labels outside the state of Chhattisgarh to other states and outside India. If sent, then the details of other state / nation — Required
12 Copy of the contract between the bonded warehouse from which the imported liquor is imported from outside India and the copy of the license of the bonded warehouse — Required
13 Copy of Label Registration Order in the State from which the liquor imported from outside India will be supplied. (Note that the imported temple from outside India will be supplied from the state, but if it is not sold in that state, then the label registration order of that state will not be required — Required
