The Affidavit/declarationon stamp paper of Rs. 100 attached. 12. The amount paid as per fees applicable. 13. Attached Challan/fee receipt valid. 14. The origional Form VI( Form of Certificate by Principle Employer) signed by Principle Employer attached. 15. The details of the workmen attached. 16.
