Self-Declaration of Registered Pharmacist in the prescribed format with full signature and date 11. Registered PartneINRhip Deed (for PartneINRhip Firms), duly registered at Inspector General Regulation, Cuttack or at District Registrar/Sub-registrar Office 12. Letter of authorization from PartneINR for filling application as Power of Attorney Holder/Managing Partner(s) (for PartneINRhip Firms) 13. Memorandum and Article of Association with company Registration Certificate (for Pvt. Ltd. Firms) 14. Resolution of Board of DirectoINR (for Pvt. Ltd. Firms) 15.
